Physiological Intracytoplasmic Sperm Injection, known as PICSI, is an advanced form of ICSI, designed to enhance IVF treatment success rates. It is designed to help identify better quality sperm before one sperm is injected directly into the egg for fertilisation. In conventional ICSI, sperm selection is performed on the basis of visual criteria. However, this may not always lead to best possible outcomes for the couple. Sperm selection in PICSI, on the other hand, is done on the basis of the physiological ability of the sperm.
The aim of PICSI is to mimic the natural process of fertilisation. In natural conception, mature sperm bind to a substance called hyaluronan found around the egg. In PICSI, sperm is placed in a special dish containing hyaluronan hydrogel. The sperm that successfully bind to it are considered more mature and functionally competent, and these are the ones selected for use during ICSI.
PICSI may help increase the chances of pregnancy by up to 20% by improving the selection of sperm used for fertilisation. By choosing sperm based on functional maturity, the technique follows a process similar to the way sperm is naturally selected in the female body during conception. In some cases, this has been associated with improved pregnancy outcomes and better fertilisation potential.
